· Position Details: For Diabetes Education program - inpatient and outpatient coverage; RNs or RDs encouraged to apply. Pump trainings, group classes, individual appointments, if RD will need to see other disease states as well. M-F
· Salary: $56K-$110K

Job Summary
Responsible for providing professional nursing, consultation and educational services for persons with diabetes and provides community health education. Develop curriculum on health, including research and provide information on the management of diabetes for patients of all ages from infancy to geriatrics.
Key Responsibilities:
- Provides education related to diabetes and the disease process; medication, diet and nutritional monitoring; footcare, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection.
- Develops and implements educational programs for allied healthcare professionals, patients and families.
- Advises patients as to long-term complications.
- Provides clinical resource and consultation to allied healthcare professionals.
- Assists patients in blood glucose monitoring for the purpose of medication control and adjustment of insulin, if indicated.
- Assesses, monitors and implements treatment plans for patients at various stages of the disease. The focus is on prevention of complications.
- Provides ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care as required.
- Documents assessments, interventions and plans including recommendations and goals, progress notes and discharge summaries.
- Communicates effectively with all team members, patients and families through conferences, individual consultation and committee participation. This includes telephone, written, and electronic communication.
- Assesses, recognizes and reports any concerns of patient abuse or neglect.
- Completes all required documentation, reports and logs in a timely, professional and thorough manner.
Qualifications
- Master's degree in Dietetics
- Bachelor's degree in Nursing preferred.
- A minimum of (2) years experience in healthcare setting, preferably in diabetes prevention or health education program.
